Physician-Directed Community Paramedicine - Not Home Health
Healthy at Home is developing a community paramedicine service, not a home health agency. Once regulatory and agency licensing is complete, licensed community paramedics will provide authorized in-home assessments, monitoring, health education, and coordinated follow-up under the direction of a licensed physician and within approved clinical protocols. Clinical services are not currently available.
A BRIDGE BETWEEN HEALTHCARE AND HOME
Our planned model extends physician-directed care into the home through community paramedics who collaborate with patients, families, primary care providers, hospitals, and community partners. The goal is to identify concerns earlier, improve follow-up, reduce barriers to care, and help people remain safe and independent in their communities.

In one matched study of cardiopulmonary patients, a 30-day post-discharge community-paramedicine intervention was associated with 40.7% fewer total emergency department visits during the 210-day study period.
